How MyCare@HOME works
MyCare@HOME is the home care app that supports both citizens and healthcare professionals, adapting to different needs with one single goal: improving the management of care.
How it works for healthcare professionals

MyCare@HOME is designed for healthcare organisations and their teams.
Each role has its own access and functions, so that work is always organised and traceable.
The organisation manager receives the access credentials and configures their team directly in the app: adds nurses and care assistants, assigns patients and manages profiles.
Each nurse supervises their working group and coordinates the assigned care assistants.
Care assistants have direct access to their assigned patients and can start assessments at any time, directly from their mobile device during the visit.
The assessment process follows a precise order based on roles:
The nurse carries out the clinical assessment: an in-depth medical evaluation across multiple body systems, which produces a frailty index for the patient. The nurse may also choose to carry out the wellness assessment in the same session.
The care assistant carries out the wellness assessment: if it has not already been completed by the nurse. This phase analyses the physical, mental and social wellbeing of the person, through structured questions, visual responses and, where necessary, graphic exercises.
Responses are saved automatically during completion. At the end, results are immediately visible with colour-coded visual indicators per area of criticality.
Based on the results, MyCare@HOME automatically suggests which care services are recommended for the patient (e.g. home care, nursing care, physiotherapy, day centre, meals at home, social worker, domestic help).
For each service, it is possible to find nearby providers directly via Google Maps, based on the location of the device in use.
At the end of each assessment, the app generates a professional PDF report including the patient’s data, the results of the wellness and clinical assessments, the history of the last five assessments and the patient’s digital signature.
The report can be downloaded, printed and shared with other professionals involved in the care.
After each completed assessment, the system automatically calculates when to schedule the next one, based on the patient’s frailty level:

The reminder is assigned to the responsible care assistant and updates automatically with each new assessment, with no manual action required. Upon expiry, the care assistant automatically receives an email notification advising them to schedule the next assessment.
How it works for citizens and families
MyCare@HOME is also available for those who want to monitor their own wellbeing or that of a family member, independently and privately, directly from home.

Simply download it, create a personal account and get started.
Enter your profile or that of your family members. You can manage multiple people from the same account, so you can monitor the wellbeing of those close to you all in one place.
Answer the questions in the guided assessment: it covers five domains (physical, cognitive, clinical, social and emotional).
The process is simple and intuitive, designed to be completed independently, even without healthcare expertise.
At the end, the app displays a clear picture of your state of wellbeing, broken down by area.
Results are shown with immediate indicators that make it easy to understand where to pay more attention.
Based on the results, MyCare@HOME suggests which services might suit you.
You can locate them directly on the map, based on your location, thanks to the integration with Google Maps.
Each assessment is saved. You can compare results over time and monitor the progress of your wellbeing, for yourself or for the person you are supporting.
