Clinical assessment home care: how it works

MyCare@HOME structures the assessment into two complementary phases, designed to be administered by the healthcare professional in a fast, standardised and time-comparable way.

Clinical Check

The first phase represents the objective component of the assessment.

Through a protocol structured in 14 clinical categories, it assesses the patient’s comorbidity burden across the same number of organ systems, assigning each a severity level on a five-grade scale.

This phase allows you to:

• assess the overall clinical complexity of the patient
• identify early conditions of organic frailty
• obtain a standardised and time-comparable score

Wellness Check

The second phase adopts a person-centred approach and is structured around five fundamental domains, each assessed through clinical evaluation tools developed internally on the basis of international scientific literature. Together, they provide a holistic representation of the state of health.

The scores collected in the two phases are automatically processed and translated into frailty levels (from “absent” to “severe”).

What MyCare@HOME returns at the end of the assessment: Upon completion of the two phases, the system automatically processes all collected data and generates a structured output immediately usable in clinical practice.

The complete report is exportable in PDF and designed to be shared with the treating physician, the multidisciplinary team or archived in the patient’s file.
